Read this first: SurgiLog is a personal record-keeping tool. It is not a medical device, and it plays no part in diagnosis, treatment, or any clinical decision.
Agreement
By installing or using SurgiLog you agree to these terms. If you do not agree, please do not use the app.
What SurgiLog is
SurgiLog is a personal logbook for recording surgical cases you have participated in and the role you held. It exists to help you keep your own professional record.
What SurgiLog is not
- It is not a medical device and is not certified as one.
- It does not diagnose, treat, monitor, or make any recommendation about the care of any patient.
- It is not a patient record system and is not a substitute for one.
- It is not a source of truth for anything clinical. Your institution’s own records are.
Your responsibilities
You are responsible for:
- The accuracy of what you record. The app stores what you enter; it does not verify it.
- Complying with your employer’s and your institution’s policies on record-keeping, and with any law that applies where you practise. The app is designed to avoid storing identifying patient information, but using it does not by itself make you compliant.
- Not entering identifying patient information into free-text fields such as Notes.
- Keeping your own backups. Your cases live on your device; if you lose the device, or delete the app, the data goes with it.
Subscriptions
Recording cases and scanning are free, with no limit on the number of cases. SurgiLog Pro adds PDF and CSV export, the statistics summary, and cloud backup when that feature is released.
- Subscriptions are billed through the App Store or Google Play, not by us.
- A subscription renews automatically until you cancel it. You cancel it in your App Store or Google Play account settings, not in the app.
- Refunds are handled by Apple or Google under their policies. We cannot issue a refund on their behalf.
- Prices may change. An existing subscription keeps its price for its current period, and any change is notified before it takes effect.
- If your subscription lapses, you keep every case you have recorded. Only export and reporting stop working.
Availability
The app is provided as it is. We do our best to keep it working, but we do not promise it will be free of defects or available without interruption. Features described as coming later may change or may not ship.
Limitation of liability
To the fullest extent the law allows, we are not liable for any loss of data, loss of records, or any indirect or consequential loss arising from your use of SurgiLog. Our total liability is limited to the amount you paid for the app in the twelve months before the claim.
Nothing here limits liability that cannot be limited by law.
Intellectual property
The app, its name, and this site are ours. The cases you record are yours; we claim no rights over them and, in any event, we never receive them.
Ending these terms
You may stop using SurgiLog at any time by deleting it. We may discontinue the app; if we do, it will keep working on your device and you will be able to export your records before any subscription ends.
Governing law
These terms are governed by the law of the Hashemite Kingdom of Jordan, without regard to conflict-of-law rules.
